CONVENTION MEMORIAL SUBMISSION

On Monday, June 16, 2025, at 5pm, the OPFFA will honour LODDs, active, and retired members that have passed since the last OPFFA convention (June 2024) at a formal memorial service. Please use the form below to add names for those we lost over the past year.

Please enter the full name, Local name, the number of years of service, the date of death, and select whether the death was a LODD, Active or Retired death. Rank is not required. Firefighters who have passed since the June 2024 convention will be honoured at this 2025 memorial ceremony.
